BodyLOVE by Morgana's / Morgana Mauney's Liability Form

Waiver and Release of Liability
I, the undersigned, hereby acknowledge that I am voluntarily participating in physical fitness training sessions with Morgana Mauney (BodyLOVE by Morgana). I fully understand and acknowledge the risks associated with engaging in physical activities, including, but not limited to, strenuous physical exertion, exercise-related injuries, and accidents.
I am aware that physical activity can involve certain risks of injury or harm, and I expressly agree to assume all such risks associated with my participation in these activities.
 
Medical History
I affirm that I have disclosed any medical conditions, physical impairments, or injuries that may affect my ability to safely participate in the personal training sessions. I will notify my trainer immediately if any condition changes or if I experience any pain or discomfort during sessions.
 
Release of Liability
In consideration for being permitted to participate in the personal training services, I hereby waive, release, and discharge Morgana Mauney (BodyLOVE by Morgana), their agents, employees, and affiliates from any and all liability for injuries, losses, damages, or other claims arising out of my participation in the training sessions, whether caused by negligence or otherwise, to the fullest extent permitted by law.
I also agree that Morgana Mauney (BodyLOVE by Morgana) is not responsible for any loss, theft, or damage to my personal property while participating in these training sessions.
 
Medical Consent and Emergency Contact
In case of an emergency, I authorize Morgana Mauney to take appropriate action and seek emergency medical care for me if necessary.
Acknowledgment of Understanding
I have read, understood, and voluntarily sign this waiver and release of liability. I understand that I am assuming responsibility for any risks associated with my participation in the personal training sessions. I acknowledge that I have had an opportunity to ask questions regarding the waiver and release, and all of my questions have been answered to my satisfaction.

I confirm that I accept and agree to all terms and conditions outlined in this waiver.
